To Open an Account
Chose an application form from among the options listed below, enter information, sign, and return the form along with a check (for any amount) for your initial investment. (The MP 63 Fund does not require a minimum investment amount.)
To make your initial investment by wire:
First, call Mutual Shareholder Services, the fund Administrator, to say that you are wiring money to your account (877-676-3386). You will be assigned an account number. If no response at that extension, dial “O” and explain that you are sending a wire to establish an account with the MP 63 Fund. You will be assigned an account number.
Then, instruct your bank to send the wire to the following address:
U.S. Bank, N.A., Cincinnatti, OH, ABA #042000013
For credit to the MP 63 Fund, Account # (enter your account number)

Your investment will be based on the closing price on the day your check is received by Mutual Shareholder Services, the Fund Administrator. Or, in the case of a wire, the closing price on the day that Mutual Shareholder receives an acknowledgement of the wired funds from the custodian bank.
